Registered Nurse- Hospice

Summary of Duties and
Responsibilities: Assigned to deliver direct care to patients in accordance with Medical Director orders and plan of care by providing support, information and skilled nursing care as needed.
Primary Department: Outpatient Services
Reporting Relationship: Director of Patient Care Services
Position Type: Non-Exempt, Full Time, Nights, Benefitted
Job Details and
Essential Functions:
oTriages after-hour and weekend calls from patients/families; performs or coordinates home visits as requested/needed
oActively participates in coordination of all aspects of patient's care, in accordance with current professional standards and practice
oParticipates in admission and ongoing patient assessments
oResponsible for developing, evaluating and adjusting the patient's Plan of Care (POC) as indicated
oContributes to patient and family counseling and education
oCollaborates with physicians and staff regarding patients' changing condition
oOversees licensed nurses and CHHA's in the delivery of patient care
oPerforms administrative functions relating to clinical records, care plans, QAPI, safety, infection control and mandatory abuse reporting requirements
oProvides support to patients/families regarding patient care service
oResponsible for establishing and maintaining a safe environment for patients/families, and a safe working environment for self and coworkers.
oParticipates in Quality Assurance and Performance Improvement (QAPI) program and Hospice sponsored in-service

Required
oSensitivity to and ability to interact with various racial, religious, and ethnic cultures in the organization service area
oAbility to remain calm with patients and their families/caregivers under fast paced, emotional conditions and to solicit sensitive information from patients and their families/caregivers
oExplain complex ideas and procedures so patients and their families/caregivers can understand the issues related to Hospice care
oKnowledge of State and federal Medicare Hospice regulations desirable
oKnowledge of terminal care criteria and end of life issues
oDemonstrate communications and interpersonal skill
oDemonstrate adaptability, organization, attention-to-detail, thoroughness, responsibility and leadership qualities
oUses independent judgment and decision-making to organize work and establish priorities
oResponsible for professional demonstration of the Hospice of the Foothills mission
oProviding accurate and timely records of critical events
oCooperation with the Governing Body, Executive Director, physicians, IDT members, and HOF CCH coworkers
oRead, write, and communicate on a professional level
oPerform multiple tasks in a timely manner

oPublic speaking, developing relationships with identified partners, groups, businesses with whom Hospice of the Foothills may have or need to establish affiliation
Education & Experience
Required
oTwo years experience as an RN in Hospice, home health, community health, or oncology unit or three years experience as an RN in a Medical/surgical area
oDemonstrated competence in Hospice care delivery techniques
oKnowledge of State and Medicare Hospice regulations
oKnowledge of terminal care and end-of-life issues
oParticipate in appropriate continuing education
oExpected to accept personal responsibility for other educational activities to enhance job-related skills and abilities, and to attend mandatory educational programs
Desired
oTwo years experience as an RN for an inpatient Hospice Unit
oBSN with public health certificate
License / Certifications
Required
oPossess and maintain current California RN license
oPossess and maintain current CPR certificate
oPossess and maintain valid California driver's license, a reliable automobile and evidence of appropriate liability insurance
Desired
oCertified Hospice and Palliative Nurse (CHPN)Estimated Salary: $20 to $28 per hour based on qualifications.
